SSM Registration
Provide your valid SSM business registration details or supporting registration document.
Food Handling Certificate
Provide proof that you have completed the required food-handling training.
Anti-Typhoid Vaccination
Provide proof of a valid Anti-Typhoid vaccination together with its expiry date.
